No. We work alongside statutory and clinical services, providing independent support that strengthens existing care arrangements. We are not a healthcare provider.

Yes. We provide post discharge support, helping patients and families understand discharge plans, organise services, and manage their transition safely.

Yes, with consent. We support clear communication between patients, families, and clinical teams to ensure alignment and continuity.

Communication & Process

Referrals can be made directly by you as professionals or via clients. We will arrange an initial consultation to assess needs and agree on the level of support required.

Yes, with client consent. We provide relevant updates to ensure all professionals involved remain informed and aligned.

Yes. We cover the whole of England and offer both one off
consultations and ongoing support, depending on the complexity and needs of the situation. Meetings and consultations can be held 7 days per week, in person or virtually.
